If the following forms are missing or found to be out of date, please proceed to the Graduate School's website for the most up-to-date forms and then alert the Graduate Coordinator Assistant.

MA (Thesis) Course Requirements Checklist

MA (Non-Thesis) Course Requirements Checklist

Advisory Committee Form [Instruction]

This form requires only the Graduate Coordinator's signature; however, all members of your committee should agree to be on your committee.

Final Program of Study Form [Instruction]

Utilize the appropriate checklist as a guide for the completion of this form. This form requires the signatures of your entire committee and the Graduate Coordinator.

Application for Waiver of Out-of-State Tuition [Instruction]

Out-of-state MA students who have completed their Final Program of Study form may apply to have their tuition reduced to in-state levels.

Thesis Defense and Final Examination Approval Form

This form will require the signatures of your Major Professor and the Graduate Coordinator.

MA Non-Thesis Comprehensive Exam Form

Used to assess the final test of the MA Non-Thesis track.

PhD with no prior related MA Course Requirements Checklist

Preliminary Doctoral Program of Study Form

Complete this form the first semester in attendance in consultation with the Graduate Coordinator.

Advisory Committee Form [Instruction]

This form requires only the Graduate Coordinator's signature; however, all members of your committee should agree to be on your committee.

Final Program of Study Form [Instruction]

Utilize the appropriate checklist as a guide for the completion of this form. This form requires the signatures of your entire committee and the Graduate Coordinator.

Qualifying Paper Assessment Form

This form is to be completed along the way in preparation of each qualifying paper. This ensures that the student has had adequate contact with their committee and the committee fully approves the document before a presentation is scheduled.

Application for Admission to Candidacy Form

To be completed at the same time as the Comprehensive Examination Form, which is provided by the Graduate School at the time of the exam. This forms require the signature of the Graduate Coordinator.

Dissertation and Final Examination Approval Form

This form will, at various times, require the signatures of your Major Professor and the Graduate Coordinator.

GRADUATION

Application for Graduation [Instruction]

An application for graduation must be filed with the Graduate School no later than Friday of the second full week (the first full week for summer) of classes in the semester of the anticipated graduation date. You may apply online via Athena.

Late Filing for Graduation [Instruction]

A graduate student who misses a graduation deadline by failing to file the Application for Graduation, Advisory Committee form, and/or completed Program of Study form will have the option of paying a single fee of $50 for late processing of all required forms.

Graduation Ceremony Info (Only PhD)

Doctoral Students who are planning to graduate should fill out the Doctoral Commencement Information in Grad Status. This will ensure your commencement information, including your dissertation title, are printed in the commencement program.

Additional Forms

Advisement Form (Linguistics-specific)

Needed every semester until you complete the Final Program of Study form for your degree program.

Academic Probation Advisement [Instruction]

Needed when a student is placed on academic probation and needs to create an advisement plan to maintain a GPA of 3.0 or higher moving forward in the program.

Request for Change of Degree Objective [Instruction]

Needed for switching between PhD, MA, and/or MA-non-thesis. Switching from MA to PhD requires 3 letters of recommendation from Linguistics faculty to accompany the form.

Leave of Absence from Graduate Enrollment [Instruction]

Needed to qualify for an exception to the graduate enrollment policy, which states that students must register for 3 hours of courses during any 2 semesters of an academic year.

Recommended Change in Program of Study Form

Needed if courses on your Final Program of Study form do not reflect the reality of the courses you took.

Defense/Exam Scheduling Form

Needed to schedule a defense or exam with the Department of Linguistics.
